A leadership logbook template is a short, lightly scored log that a regional head or senior leader fills when visiting a store, so the visit leaves a record of what they saw and heard, one thing to fix and one thing to celebrate. It is deliberately not an audit. Four entries are scored on a scale of 1 to 5 and the rest are short notes, with a pass mark of 80 and no fatal items. The leader fills it on the phone in about 15 minutes, adding a visit photo with GPS and time stamp. The area manager reviews each entry, and the one thing to fix becomes a ticket with an owner and a deadline, so the leader's observation turns into action rather than a comment that fades after the visit.

Score = weight of items answered Yes ÷ weight of items not marked N/A, out of 100. Pass mark 80. A failed fatal item caps the score at zero and raises a ticket to the area manager.
